If you are 40 to 74 years old, you can get a free health check.

This check helps find out if you might get:

• Heart problems

• Diabetes

• Kidney problems

• Stroke

Your doctor will:

• Ask you some questions

• Check your blood pressure

• Check your weight and height

• Take a small blood sample

This helps your doctor see how healthy your heart is.

You can get this check if:

• You are 40 to 74 years old

• You don’t already have heart problems, diabetes, or kidney disease

• You haven’t had a health check in the last 5 years

• You are not already taking medicine for high blood pressure
