How to refer

In designing the pathways into this service, we wanted to ensure that it was simple and easy for people to gain access to the service, and, in turn, the falls prevention plan.

It is our ambition to ensure residents have access to a falls prevention plan within Hartlepool.

Who can refer?

Anyone can refer into the service.  This means:

  • an individual can refer themselves in
  • a carer can refer on someone's behalf
  • any professional can refer in

How do I refer?

Referrals are simple, just download the screening tool, complete and return to the team via a variety of methods:

  • Post to: Falls Prevention Team, Ward 8, University Hospital of Hartlepool, Holdforth Road, TS24 9AH

If you would need assistance in completing the form or more information on the service, contact the team on 01429 401807 and they will be happy to assist you.

What can I expect from the Falls Prevention Service?

A representative from the service will contact you, discuss your referral, provide you with advice/information/signpost you to appropriate service or arrange a home visit to complete a falls prevention plan.

We will be in touch by 28 days from receiving the referral.

Why should I have one?

If you've fallen in the past, making changes to reduce your chances of having a fall can also help you overcome any fear of falling.

Some older people may be reluctant to seek help and advice from their GP and other support services about preventing falls, because they believe their concerns won't be taken seriously.  However, all healthcare professionals take falls in older people very seriously because of the significant impact they can have on a person's health.
