Description of Service
The service is for adults 18+ with a diagnosed mental health need who are a resident of Hartlepool.
Choices for Life is an innovative day service for adults with mental health needs that puts participants at the heart of their decisions. As part of Creative Support, a nationwide not for profit provider of personalised services we encourage and support project members to access a broad rang of in house sessions, one to one support and community based activities and events. We work in partnership with a range of services across Hartlepool.
At the centre of our service is our social enterprise, Creative Cafe, a community hub open to the general public that sells delicious home cooked food. The Cafe also has an exciting selling and exhibition space and offers voluntary opportunities allowing individuals to get work experience, learn new skills and gain qualifications. We run a broad range of sessions in house that are open to the general public as well as our project members.

We accept referrals from a variety of agencies. This includes statutory services, GPs and other agencies that work with adults who have diagnosed mental health needs. For more information or to request a referral form please contact Choices for Life.
