Description of Service
Avalon is a social care charity. We have delivered care and support services to people across the North of England for the past 20 years. We currently support around 800 customers with a range of support needs, including: acquired brain injury, autism, challenging behaviour, complex needs, dementia, learning disabilities, mental health issues, older adults, physical disabilities, sensory loss. Our mail services include Supported Living, Community Support and Shared Lives. Supported Living provides care and support services to individuals to help them to remain living in their own homes, maintain and develop independence. Community Support provides individuals with care and support to undertake leisure, social, educational and work activities in their community. Shared lives enables people with care and support needs to share the home of a fully trained carer as part of their Shared lives family. We are an established local social care provider - we currently deliver care and support to over 150 customers across Teesside. Local delivery activity is managed from our office in nearby Stockton. Within Hartlepool, we are members of the Council's Framework for Provision of a Support Service for People with Autistic Disorders. As part of the Framework, we can offer care and support services to help people with autism to live independently and participate in activities in the community. We also offer care and support services to privately funded customers across Teesside and have recently launched a dementia specific care and support service.

Is there a cost for this service?
Costs are dependent upon factors such as the location, time of day, nature and complexity of support required. If you are a member of Hartlepool Borough Council's Social Care Team and wish to instruct us to deliver provision through our membership of the Framework for Provision of a Support Service for People with Autistic Spectrum Disorders please refer to the pricing information which is detailed within our contract to deliver this service. Our office is open from 8:30am - 5pm Mon - Thurs and 8:30am - 4:30pm on Fri. Outside of these hours, Avalon operates an on call service for Avalon customers.

We provide practical care and support to help individuals to meet their religious and cultural needs. So, for example, this might include accompanying individuals to places of worship, assisting them to shop for and prepare food in accordance with their religious practice. All of our support workers undertake Equality and Inclusion Training as part of their induction. Within our Teesside service, we also employ an Arabic speaking support worker, who can provide verbal translation support.
