Description of Service

Extra Care: Laurel Gardens is an Extra Care scheme for customers over the age of 55+ with an identified care need. The scheme itself is on Marlowe Road, Rift House which is on a main bus route, close to shops, post office and the local club. There are 60 two bed roomed apartments of which 30 are for social rent, 15 for outright purchase and 15 which are shared ownership. All apartments are open plan living, with living room/kitchen, separate bedrooms and bathroom with level access bathing facilities. Six of the apartments are fully adapted, with rise and fall kitchen units. There are 2 lifts to the upper floors and an assisted bathroom on the first floor. Whilst there is the space in each apartment for a washing machine we also have a communal laundry facility available to our residents. There is an intermediate flat on the top floor which is owned by Social Services and used to support short term reablement care and support for a period of up to six weeks. This is monitored by the support officer on site and communication with the Social Worker involved in the care and support. The scheme also benefits from a Bistro open daily, Hairdressing Salon, communal garden room, lounge and sitting area. There are also outside gardens and greenhouse. The scheme also benefits from a guest suite situated on the top floor for which residents can access for a nominal charge for their families and friends. There is a scheme manager on site from 8:30am until 5pm Monday to Friday who can offer advice and support. Stay Independent: all apartments have access to pull cords and each individual offered a pendent to call for assistance in emergencies. 24-hour support: there is care provided onsite 24 hours a day, 375 days a week which is provided by Comfort Call. They are commissioned to provide care and support through a Social Services Support Plan and Assessment, which is accessed through Hartlepool Borough Council Social Services. Outside agencies: we have good links and communication with other support providers and Hartlepool Borough Council so the right care can be accessed when identifying individual needs. Community: use of all on-site facilities and activities. We try to create sociable and friendly community.

Costs

Rent and service charges can be discussed on an individual basis. If you are in receipt of care you will be assessed through Adult Social Services and advised what contribution you will pay towards the cost of the care received.

Opening Times

Laurel Gardens is open to the general public seven days a week, if you would like a tour or to view this scheme please contact the support officer on site 01429 868884 between the hours of 8:30am - 5pm. There is a Bistro on site which is open to the public seven days a week from 9am - 3:30pm and there is a car park to the rear of the building.

The building is wheelchair accessible, hearing induction loop within the building, the building has a fob access and door entry with camera protection to cover for secure doors. We can provide support, advice and access to adaptations for each tenants individual needs communal areas are also accessible for our tenants diverse needs.

Sign Language Class on a Tuesday Afternoon. There are staff within the organisation who can sign and also interpret and all information is provided in many languages Braille and also audio.

The Bistro can cater for diabetics and also vegetarian options. We have and can refer into services when requested by our residents to arrange for them at their request to access holy communion in their own homes.

Compass application form and a referral via Social Services SDAQ and support plan to access. All referrals are via Hartlepool Borough Council's Social Services Department and as vacancies arise they are assessed on the needs of both the applicant and the care/support needs within the scheme.
