Description of Service
Across the North East of England, Care Visions at Home provide high quality personal and clinical care services to support individuals to remain living in their own home environment. This service is provided by a local, highly trained and skilled staff team who safely deliver specialist health and social care support to children and adults of all ages - 24 hours a day and 7 days a week. A member of our nurse team can clinically assess individual's needs, and train health care assistants with the required specialist skills to care and support service users. Our specialist staff are trained and signed off as competent by our nurse team to provide the following range or personalised care: all aspects of personal care (including continence and catheter care), medications support (including pain management), pressure care support, nutritional support and hydration (including enteral feeding), oxygen and ventilation support, tracheotomy care, support to access the local community. Our specialism is working with service users who have complex health and social care needs who want to remain living in their own home environment, or be supported in interacting in activities within their local community. The Teesside and Durham branch staff team provide a person centred assessment and care planning/support service which identifies all of the individual's needs and outcomes, alongside understanding the breadth and depth of their condition. We have experience of supporting service users whom have multiple health needs and/or profound and intense conditions, and include those adults and older people with: neurological conditions including dementia, physical disabilities who have restricted or no movement, palliative and end of life care for service users with terminal illnesses/conditions, acquired brain injuries who can present varying levels of challenging behaviour and who require 24/7 care and support, rare and complex genetic conditions/diseases who require varying degrees of 1:1 complex care. With these health issues in mind, we provide bespoke training for our health care assistants. A member of our nursing team leads, and works with the operations team to assess service users and prepares specific, appropriate staff training and competency based assessments. This ensures that the team of staff who are placed around the service user can provide the appropriate level of high quality, specialised care and support across the locality.

Hourly charge rates:
£15.50 non clinical care
£16.50 clinical care
We provide a 24/7 service, 7 days per week. Outside of branch opening hours (Monday - Thursday 9am - 5pm, Friday 9am - 4:30pm), we provide a support and guidance service out of hours, 7 days per week for our clients. This dedicated service provides assistance from the on call team. In cases of emergency the branch manager is also available for support and advice. When providing clinical care for clients staff can also access specialist advice from our nurse team across the company.
