NHS Community Services

The NHS community specialist palliative care team provide on-going specialist advice, support and review of patients who have complex specialist palliative needs to people in their own homes and in care homes. The team includes clinical nurse specialists, physiotherapists, occupational therapists and a specialist therapy technician. The community specialist palliative care team provide on-going specialist assessment, advice, treatment and holistic support to assist patients to achieve and maintain their maximum physical, emotional, spiritual, vocational and social potential. The team liaise closely with patients GPs and district nursing teams, and provide specialist input in regards to effective symptom management, advance care planning, holistic support and specialist therapy input. Our focus is providing advice so the main Carer can manage the patients’ problems effectively. They sometimes make short-term interventions relating to specific unresolved problems. Their intention is to discharge the patient from the service back to the referrer when the patients’ needs (in the relation to their problems) have been resolved. In some cases, some patients will remain under their care for a longer period or throughout the disease process. Typically, patients needing this level of intervention have multiple conditions that need specialist input, or will have been referred at a late stage of their disease. The team will also help the primary Carer or key worker in assessing and managing the needs of the patient and Carer. They will not take over care but will act as a specialist resource. The services is available from 9am to 5pm Monday to Friday (excluding bank holidays). Outside these hours please contact your GP or community nurse.
