Service summary
Community Services Team
Service will provide members of the community with support that are at risk of homelessness.
The service will provide support in locations agreed between the support worker and the service user, this can include the service users property or within the community. The provider will provide support flexibly and the hours allocated will cater for the changing needs of individuals and support will be provided at flexible times to meet individual need. The service will be delivered flexibly between 9am and 5pm Monday to Friday.
The service works in partnership with statutory and voluntary agencies to ensure service provision is effective, focused and responsive to local needs.

Home Group have documents accessible in all languages. Interpreters can be utilised through our many services. All religious and cultural needs are catered for as all clients receive person centered planning and support.

All clients or external agencies can contact service via phone 03001310037 or email scottgrange@homegroup.org.uk. All referrals are then followed up with initial assessments to ensure we can meet the needs of the individual before acceptance.
