Description of Service
HART (Hartlepool Action Recovery Team) consists of two services. Addaction provides the clinical interventions including substitute prescribing where appropriate and HBC (Hartlepool Borough Council) offer support via Neurological, Biological, Psychological and sociological interventions. The aim of the service is to build resilience and support in their own communities so that clients have that continued support when they leave substance misuse services.
HART young people service takes referrals up to the age of 18 who have substance misuse issues. Referrals are accepted by telephone, letter and in person, from voluntary/professional organisations, schools, social workers and individual within Hartlepool. During referral stage we discuss what support HART can provide and make a further appointment to complete a comprehensive assessment.
Once a referral is made to the team, the young person will be allocated a young people’s support officer. The comprehensive assessment may take up to 6 weeks, to ensure the team can build positive relationships with the young person. At assessment stage we provide:
• Confidential service
• Harm minimisation advice and support
• Discuss treatment options available
• Identify Physical, psychological & social needs
• Signpost to onward agencies if identified need.
After assessment the team will work alongside the young person to meet individual needs. I.e. offering a one off session or several, within these sessions the team will offer reduction techniques harm minimisation, advice etc... This can be structured or unstructured depending on the young person’s need.
The young people’s team can offer group sessions within schools and so forth upon request.
The young people’s team work closely alongside the youth offending service, schools, youth clubs and offsite provisions (PRU/ACE/BIP). The team also offer substance misuse education to the wider community (teachers, Parents/Carers, social workers – dependant on the need).
The young people’s team offer a drop-in service once a week within the One Stop Shop (Middleton Grange Shopping centre), this drop-in offers information, advice and guidance, employment opportunities and support from the sexual health team.
