Community Connector - North
Each Community Hub has a friendly, approachable Community Connector that can help you to access the support you need to improve your physical and mental well being, live more independently and become more involved in your community. We're here to help you to:- * Take simple, effective actions to improve your health and well being. * Find new ways to tackle a problem or concern. * Gain skills and confidence to open new doors and opportunities. * Find out more about groups, activities or events. * Make new friends and widen your social network. Each Community Hub runs a short health walk and social groups where local people can come together to walk out into the surrounding area and then enjoy free refreshments/games/ health information and general chat. These groups run independently and you do not have to access the walk to access the social groups and vice versa. We can sign post you into a huge variety of activities available within Hartlepool Borough Council and the wider community. If you would like to find out more please contact us on the details below.

There is no cost to access a Community Connector, however, different activities usually incur a small fee. Concessions available via Active Card Scheme.
