If you or someone you know needs advice or support, Hartlepool Support Hub is here to help you stay safe, stay well and stay connected. 

 

 

The Support Hub offers:

The Hartlepool Support Hub can help signpost you to the services that you want, utilising local knowledge and resources. Being able to help you to live independently and empower you to make the most of what's available, The Support Hub is the first point of contact for Adult Social Care within Hartlepool. The Team are able to talk to you around the everyday things that are important to you so that you can be in control of your own life. Pick up the phone or drop into a community hub and lets start that Good Conversation. 

 

Wellbeing Service 

The Wellbeing Service is part of the wider support hub offer, which provides the opportunity for residents to learn about and take part in a variety of supervised exercise and activities which can support better self-management of a wide range of long term health conditions.

There are various different options available ranging from group water based therapeutic exercise, circuit based sessions to individualised 1:1 sessions that can enable a greater understanding of the benefits of a personalised approach to staying as well as possible.

At both of our main Community Hub sites we have specialist MOTOmed generated cycles that can accommodate all abilities, including those who are wheelchair users.

Supported Internship 

The team also coordinate the Supported Internship programmes, which consists of Project Search and the Valuing Individuality Programme (VIP).

The aim of these programmes is to empower and raise the aspirations of young adults with additional needs, with the aim to help them gain either independence and employment or volunteering opportunities within their local community.

Supported Internship works in partnership between The Support Hub and Jobs & Skills.
